Don Henderson founded Care 4 U Management, Inc. in 2014 to provide case management and support services to individuals and families affected by HIV epidemic in Miami-Dade County – specifically, the disproportionately impacted area of Liberty City. Care 4 U started providing services with funding from the now-defunct Medicaid Waiver Project AIDS Care (PAC Waiver) program in the heart of Liberty City, on NW 27th Avenue and 66th Street. Soon after PAC Waiver was sunsetted at the end of December 2017, Don partnered with Vanessa Mills, co-founder of Empower U, Inc., to expand the services Care 4 U provided to the community. The agency began providing outreach and HIV/STD testing to identify and connect newly diagnosed and those lost to care to health and support services. 

In June 2018, a second location was opened on NW 7th Avenue and 47th Street, to provide medical care.  With this new location, the agency grew at a tremendous rate. Care 4 U then began its journey to become a full-fledged primary health care center. 

Today, Care 4 U's operates with the mission to ameliorate barriers to accessing primary care by expanding medical care services to reach low-income individuals regardless of their ability to pay. The health center provides primary medical care for men, women, and children, including well-child visits; immunizations; HIV specialty care; and STD screening. Care 4 U also provides outreach, case management/linkage to care services, on-site pharmacy, mental health and substance abuse counseling, transportation and emergency financial assistance services to low-income individuals in the high-need area of Liberty City.

Care 4 U Community Health Center is a comprehensive community health center, providing a full range of health care and preventative services to our community’s adult, adolescent, pediatric and specialty needs populations.

Our Services

Our services include the following: 

Primary Medical Care Services

These services include: adult medical care, women health care, pediatric health care, transgender and same sex loving, behavioral health and substance abuse and nutritional services.

Other Services

These services include: medical case management, linkage case management, oral health/dental, service referrals, condom distribution and Pre-Exposure Prophylaxis (PrEP)/Npep services.

STD Screening Services

These services include: HIV testing and counseling, sexually transmitted Infections and screenings and Hepatitis C (HepC) screening.

Additional Services

These services include: early intervention services, emergency utility assistance, Florida AIDS Drug Assistance Program (ADAP), sliding scale fee for services and navigators/certified application counselors (Obama Care).
