About Us
Care 4 U Community Health Center, Inc. (C4UCHC) is a Florida-based nonprofit 501(c)(3) minority organization founded in 2013. Our mission is to address the unmet health and social needs of individuals and families affected by HIV/AIDS. Our vision is to ensure equitable access to care for all people living with HIV/AIDS and to eliminate new infections.

Originally established as a medical case management agency for People Living with HIV (PLWH), C4UCHC has evolved into a comprehensive Medical Clinic.
In 2018, we launched a strategic expansion plan to broaden our services and clientele. By May, C4UCHC became a Florida Department of Health (FDOH)-registered HIV Test Site, offering prevention counseling, testing, and linkage-to-care services. Soon after, we partnered with FDOH to establish an STD Clinic. In June, we expanded to a second location and began providing primary care services.
Now a full-fledged Community Health Center, C4UCHC delivers a wide range of healthcare and preventive services, dedicated to reducing health disparities and improving health outcomes in the communities we serve.

Our Board of Directors is a dedicated team of professionals who guide our mission to deliver high-quality healthcare and social services, with a special focus on those affected by HIV/AIDS. Their diverse expertise has been instrumental in transforming us from a medical case management agency into a comprehensive Community Health Center. Their commitment fuels our continued growth and dedication to providing compassionate, accessible care to our community.
