We are a comprehensive community health center, providing a full range of health care and preventative services to our community’s adult, adolescent, pediatric and specialty needs populations.
Oral Health and Dental Service Referrals

Our Case Managers coordinate referrals through agreements with Federally Qualified Health Centers (FQHC) for specialty care, emergency services, dental examinations, cleanings, x-rays, fillings, prosthetics, and gum disease treatment.
Medical Case Management Services
​
We provide referrals, low-income eligibility assistance, and a range of community support services. Our Medical Case Management team helps clients develop and adhere to their healthcare plans.
Linkage Case Management Services
​
We assist clients in establishing long-term connections to medical care and support services. Our Linkage to Care Coordinators help with benefit applications, including Affordable Care Act insurance, ACCESS Florida, and linkage to HIV primary care and other benefits.
Pre-Exposure Prophylaxis (PrEP)
​
PrEP, used with condoms, is a prevention option to reduce the risk of HIV infection through sexual contact. If you're at risk for HIV, talk to your doctor about PrEP.

We also offer nPEP for those who may have been exposed to HIV. nPEP should be taken within 72 hours of exposure for it to be effective.
Condom Distribution
​
We provide condoms to the community to help reduce the transmission of STIs such as chlamydia, syphilis, gonorrhea, and HIV.
