Patient Responsibilies
As a patient at C4UCHC, you have certain responsibilities that are essential to ensuring you receive the best care possible and contribute to the success of your treatment. Your responsibilities include:

Respect: You are responsible for treating all staff, volunteers, and fellow patients with respect.

Compliance: You are responsible for following the rules and regulations of C4UCHC that affect patient care.

Confidentiality: You are responsible for maintaining the confidentiality of other patients.

Grievance Procedures: You are responsible for following the client grievance procedures.

Appointments: You are responsible for keeping your appointments. If you need to miss an appointment, please notify the health center with 24 hours' notice and reschedule.

Accurate Information: You are responsible for providing accurate and complete information regarding your current complaints, past illnesses, hospitalizations, medications, and other matters related to your health.

Coordination of Care: You are responsible for informing providers at other agencies from which you receive services that you are also receiving care from C4UCHC. This coordination of services between agencies benefits your overall care.

Reporting Changes: You are responsible for reporting any unexpected changes in your medical condition.

Treatment Plan: You are responsible for following the treatment plan you developed with your provider, including understanding the planned course of treatment, alternatives, risks, and prognosis.

Consultation: You are responsible for allowing your provider to consult with other healthcare professionals about your care.

Treatment Compliance: You are responsible for the consequences of refusing treatment or not following your provider’s instructions.

Financial Responsibility: You are responsible for arranging payment of medical bills and applying for all benefits and entitlement programs for which you are eligible.

Sobriety: You are responsible for remaining drug and alcohol-free while on the health care facility premises.

Transportation: You are responsible for providing your own transportation whenever possible. If you are unable to do so, you should contact your provider.
Patient’s Bill of Rights and Responsibilities

The Patient’s Bill of Rights and Responsibilities is designed to promote the interests and well-being of patients and to enhance communication between the patient and healthcare provider. Florida law requires that we recognize your rights while you are receiving medical care at our center and that you respect our right to expect certain behaviors from you.
