Appointment Expectations

At Care 4 U Community Health Center, we are dedicated to providing quality healthcare to all our patients with respect and dignity. To ensure prompt and efficient service, please arrive at least 20 minutes before your scheduled appointment to complete any necessary paperwork. To receive the best possible care, please follow these guidelines for your upcoming visit:

 

Arrival and Appointment Duration

  • Arrival Time: Please arrive at least 20 minutes early to complete all necessary paperwork.

 

  • Appointment Length:

 

  • ​​Your initial medical appointment will last about one hour to ensure an accurate diagnosis and a care plan tailored to your needs.

​​

  • Follow-up appointments typically last 30 minutes or less.

 

Preparing for Your Visit

​

Before your visit, we will have received your healthcare records from your primary care provider or referring doctor. However, it’s important to also bring the following items with you:

​

  • Advance Directives: A copy of your advance directives, if you have one.

 

  • Allergies: Any known allergies.

 

  • Test Results and Lab Reports: Copies of any test results or lab reports (ask your primary doctor for these).

 

  • Surgical History: Dates and locations of any surgeries or procedures.

 

  • Medical History: Dates when your health problems began, and treatments received.

 

  • Family Health History: Information on conditions your parents, grandparents, or siblings have or had.

  • Medications: A list of all medications you are currently taking, along with the conditions they are prescribed for.

 

  • Doctor Contact Information: Names, addresses, and phone numbers of other doctors treating you, who referred you, and whom you want to receive our consultation report.

 

  • Personal Health Notes: Any notes about your personal health history.

 

  • Identification: Picture identification (e.g., driver’s license, passport, etc.).

 

  • Supplements: Any over-the-counter medicines, herbs, or vitamins you are taking.

 

  • Symptoms: A list of symptoms you’ve experienced prior to this visit.

 

  • Vaccination Records: Dates and locations of vaccines you have received.

 

  • Insurance Information: Your insurance card and insurance referral authorization.

 

  • Social Security Number.

 

Additional Tips for a Successful Visit

  • Bring Your Questions: No question is a bad question. Write down any questions you have, and ask your family member or loved one to suggest questions as well. This helps ensure you remember to ask and get the answers you need for your care.

 

  • Bring Your Medications: On the day of your visit, bring all your medications in their original containers. Your doctor will review them with you as part of your overall care.

 

  • Bring a Family Member or Friend: Bringing a trusted person to your appointment can help reduce stress, provide support, and assist in remembering questions and information discussed during your visit.
