It is our policy to ensure access to care in a timely, culturally sensitive, non-discriminatory manner and to minimize disruptions in the delivery of your care. To make your visit a more pleasant and faster experience, we have provided the patient forms below that you may print, fill out and bring in with you to your appointment. We encourage you to arrive 30 minutes earlier than your scheduled appointment time.
