Mission and Vision

Care 4 U Community Health Center, Inc. (C4UCHC) is a Florida, nonprofit 501(c)3 minority organization. It was founded in 2013 with a mission to fulfill the unmet health and social needs of individuals and families infected with and affected by HIV/AIDS. The ultimate vision of the organization is access to care for all people living with HIV/AIDS and the elimination of new HIV infections. Originally a medical case management agency for PLWH, in less than five years, the organization has expanded to two locations, including a Medical Clinic.

In March of 2018, C4UCHC embarked on its strategic plan to expand services and clientele. In May 2018, C4UCHC became a Florida Department of Health (FDOH) registered HIV Test Site to conduct HIV Prevention Counseling, Testing and Linkage to Care Services. Shortly after, the agency entered into a contract with FDOH to also become a STD Clinic. C4UCHC further expanded its services when it opened a 2nd location to provide primary care services in June 2018. We are now a Community Health Center, providing a full range of health care and preventative services to help reduce health disparities and improve health outcomes in the community we serve.

Our mission is to fulfill the unmet needs of individuals and families to build a healthier community.
