Other Services

We are a comprehensive community health center, providing a full range of health care and preventative services to our community’s adult, adolescent, pediatric and specialty needs populations.

Oral Health Service and Dental Service Referrals

Referrals are coordinated by our Case Managers through our agreements with other Federally Qualified Health Centers (FQHC). 

 

The referrals are made for specialty care, emergency services, dental care examinations, cleanings, x-rays, fillings, prosthetics and treatment of gum disease.

Medical Case Management Services

Services include referrals, low income eligibility and a wide variety of support services in the community to help assist clients with meeting their needs. Medical Case Management plays an essential role in assisting clients with developing and adhering to a health care plan.​

Linkage Case Management Services

Linkage Case Management Services

Services include assisting clients to establish a long term connection to medical care and additional support services. 

 

Our Linkage to Care Coordinators help clients with benefit applications such as the Affordable Care Act insurance applications, ACCESS Florida applications, linkage to HIV primary care and other benefits.

Pre-Exposure Prophylaxis (PrEP)

PrEP, along with condoms use, is a prevention option to reduce the risk of becoming infected with the HIV virus. The goal of PrEP is to prevent HIV infection if you are exposed to the virus through sexual contact. If you are at risk for getting HIV please speak to your doctor about PrEP.

 

We also provide nPEP. nPEP may be for you if you engage in a behavior that has placed you at risk for acquiring HIV infection. If you think you have been exposed to HIV you must take nPEP within 72 hours of the exposure for the medication to work.

Condoms

Condom Distribution

Care 4 U Community Health Center provide condoms to the community. It is important to increase condom availability to reduce STI transmission like chlamydia, syphilis, gonorrhea and HIV in the community.
