Mission and Vision
Care 4 U Community Health Center, Inc. (C4UCHC) is a Florida-based nonprofit 501(c)3 minority organization, founded in 2013 with the mission of addressing the unmet health and social needs of individuals and families affected by HIV/AIDS. Our ultimate vision is to ensure access to care for all people living with HIV/AIDS and to eliminate new HIV infections.

Originally established as a medical case management agency for People Living with HIV (PLWH), C4UCHC has grown significantly in less than five years. Today, we operate two locations, including a Medical Clinic, offering comprehensive healthcare services.

In March 2018, C4UCHC initiated a strategic plan to expand our services and reach a broader clientele. By May 2018, we became a Florida Department of Health (FDOH) registered HIV Test Site, providing HIV Prevention Counseling, Testing, and Linkage to Care Services. Shortly thereafter, we partnered with FDOH to become an STD Clinic. Our expansion continued with the opening of a second location in June 2018, where we began offering primary care services.

Now a full-fledged Community Health Center, C4UCHC provides a wide range of healthcare and preventive services, aimed at reducing health disparities and improving health outcomes in the communities we serve.
